Helping you and your family reach and maintain good health is very important to Credit Union One. Credit Union One is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Full-time employees working at least 30 hours per week are eligible to participate in benefits offered by Credit Union One on the first of the month following 60 days of employment. When you enroll in the benefits program, you may also cover your eligible dependents. Your legally married spouse is eligible for the medical, dental, vision and voluntary coverages.
Below is a break down of dependent children eligibility requirements:
- Medical: Last day of the calendar year the child turns age 26
- Dental & Vision: Last day of the calendar year the child turns age 19; or 25 if full time student
- Supplemental Life: Last day of the month the child turns age 25
Part-time employees are eligible for Teladoc and Employee Assistance Program only.
ENROLLMENT
For new hires, your elections will be effective the first of the month following 60 days of employment and will remain in place until December 31, 2022 as long as you remain an eligible employee. However, if you have a "Qualifying Events" such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan, you may make changes to your elections within 30 days of the event. In order to make changes, please access your UltiPro portal.

MEDICAL/RX
BCBSM will continue to be your medical and prescription carrier for 2022 with no changes to your benefit plans.

TELEMEDICINE
CUO offers all employees access to virtual care through Teladoc. Teladoc provides members the opportunity to receive medical care from the comfort of your own home. With Teladoc, eligible employees and your family members have access to board certified doctors who can diagnose, treat, and prescribe medication 24/7 365 days of the year from anywhere.

DENTAL
Credit Union One's dental plans provide comprehensive coverage to help you and your family maintain good dental health. Seeking care in the Delta Dental PPO Network will give you the lowest out of pocket expenses no matter which plan you select. The Enhance Plan will also provide coverage out-of-network, however, your costs will be higher.

VISION
DID YOU KNOW?

- Babies should receive their first professional eye exam at 6 months
- 80% of learning in the first 12 years comes through the eyes
Credit Union One offers vision coverage through National Vision Administrators (NVA) to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide.

LIFE & DISABILITY
New York Life | NYL (Previously known as Cigna)
Credit Union One provides eligible employees with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability through Unum.
Basic Life/AD&D
Employees working 30 or more hours per week are eligible for 1 times your annual earnings not to exceed $500,000 payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable in the event of a covered dismemberment or if your death is the result of an accident.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short Term Disability (STD)
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Credit Union One covers a portion of the costs associated with the STD coverage, however, all eligible employees are required to contribute $2.00 per pay period to cover a portion of the costs. STD benefits will begin on the 15th day of a covered absence from work and will replace 60% of your covered weekly income to maximum benefit of $3,500 per week. These benefits are payable for a covered claim for up to 11 weeks.
Long Term Disability (LTD)
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Credit Union One covers the cost of your LTD insurance, however the expenses associated with paying for the insurance is reported as income to you in order to allow for a tax free benefit in the event of a claim. LTD benefits will begin on the 91st day of a covered claim and will replace 60% of your covered monthly income to maximum benefit of $20,000. These benefits are payable for a covered claim until your social security normal retirement age.
Family Medical Leave (FMLA)
In the event that you have an approved disability for an approved leave of absence, you may also qualify for Family Medical Leave (FMLA). FMLA will protect your position with CUO for up to 12 weeks if you are unable to work do to the following reasons:
- The birth or adoption or foster care of a child
- A medical condition of your own
- To care for a spouse, child, or parent who is experiencing a medical condition

FSA
The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care FSA
- Up to $2,750 annually
- All benefit eligible can participate
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Dependent Care FSA
- Up to $5,000 annually
- Limited to $2,500 if you are married and file separate tax returns
